Medical Form Instructions from the School Nurse
To download the medical forms, it's best to use a computer instead of a mobile device. When you click the link, you will see a download icon at the top right of the page next to the print icon (see the arrow in screenshot below*). You will download a folder containing the various fillable pdf forms.
